● Comprehensive Reporting: We provide a detailed report outlining your support needs and independence levels for funding purposes.
● Face-to-Face Assessment: The process begins with a direct initial assessment to gather all necessary information.
● Time Allocation: An FCA typically requires 14 to 18 hours of total billable time, which covers the face-to-face assessment, travel, and extensive report writing.

Nurse-Led Continence Assessments

What is a Continence Assessment?

At Bridging Potentials, these assessments are led by Johanna Alejo, a Registered Nurse with extensive experience in clinical care and disability services. These assessments result in a person-centered plan that considers clinical needs, skin integrity, lifestyle, and environment to help individuals live safely and with dignity.

What are the costs and requirements?

Cost: This specialized assessment is provided at a flat rate of $119.00.
Duration: Sessions typically last between 1 and 2 hours and can be held at the clinic or your home.

Nursing Support Services

Wound Care Nursing Support

What is it?: Our experienced nurse provide comprehensive wound care to promote healing and prevent complications.
What is included?: We assess wounds thoroughly, create personalized care plans, and use evidence-based techniques to dress and manage both acute and chronic wounds.
Importance: By educating clients and their supports on skin integrity, pressure management, and signs of infection, we help optimize recovery and enhance comfort.

Stoma Care & Management

Who is this for?: We provide routine care for clients living with colostomies, ileostomies, and urostomies.
What is included?: Support includes regular appliance changes, basic stoma site care, and monitoring of skin integrity.
Goal: We assist with established care routines to promote independence while identifying concerns that may require a specialist referral.

Diabetes Management Support

What is the focus?: Our nurse provides holistic support centered on safe blood glucose monitoring and insulin administration.
What is included?: We work with participants to tailor care plans addressing diet, activity levels, and medication timing.
Risk Management: We focus on managing hypoglycaemia and hyperglycaemia risks to improve overall health outcomes.

Enteral Feeding Management

What is it?: Our team provides reliable support for clients requiring enteral feeding, including feed administration and routine tube care.
What is included?: We manage site monitoring and flushing procedures in line with plans from dietitians and medical specialists.
Education: We educate clients and carers on hygiene, equipment management, and recognizing signs that require clinical review.

Medication Management

What is included?: We support clients with safe medication administration and the establishment of routines to promote adherence.
Monitoring: Our nurse monitors effectiveness and communicates with GPs or pharmacists as required.
Note on Reviews: We do not provide formal medication reviews, which are conducted by pharmacists; our focus is on safe administration and reducing the risk of missed doses.

Occupational Therapy (OT) & Ongoing Support

What can an Occupational Therapist help me with?

Our OT team provides targeted support in areas such as developing routines for daily living skills (cooking, dressing), assessing assistive technology (wheelchairs, specialized seating), and recommending home modifications like ramps or rails. We also provide dedicated pediatric support for developmental and sensory needs.

What are your rates and session lengths?

Hourly Rate: Our standard Occupational Therapy rate is $193.99 per hour.
Initial Assessment: We allocate 1.5 hours for the first visit to ensure a comprehensive gathering of information.
Ongoing Sessions: Standard appointments are 60 minutes, consisting of 50 minutes of direct therapy and 10 minutes for session notes and feedback.

Funding, Travel & Logistics

Are you NDIS registered?

Bridging Potentials is not yet NDIS registered. Therefore, we currently service self-managed and plan-managed participants only.

Self-managed: Invoices are sent directly to the client.
Plan-managed: Invoices are sent to the Plan Manager, who then provides you with remittance advice.

Do you charge for travel?

There are no travel fees for sessions held at our clinic. For mobile sessions (home, school, or kindergarten), travel is charged as per the NDIS price guide, with a cap of 30 minutes at the $97 per hour rate.
